Onboarding

360° view of a patient’s health history.  Includes patient-generated data such as Remote Patient Monitoring (RPM), Social Determinants of Health (SDOH) and Interactive Health History Form.  The first step is to identify the data sources you want available in Patient Lookup.

Access Methods:

  • EHR API or SMART FHIR API (access data in EHR). Request setup assistance or follow specific EHR instructions on this page.  If using the EHR API or SMART on FHIR API, your IT team will have very nominal steps to complete.

  • Go to Patientlookup.com and sign in using credentials.

  • Search for patient by name in the Navigation Bar.

  • Single Sign-On (SSO)

How to sign-up for Patient Lookup in Epic?

 

Click the link below and send an interested organization request.​

 

 

 

 

We will then receive an email notification and your request will appear on our Interested Organizations page.

 

After you’ve completed a Patient Lookup license agreement, we will issue keys to you. It can take up to 12 hours before the keys are available in your Epic system.

 

Clients must express interest through the App Orchard website. This process ensures your request appears on our Interested Organizations page so we can issue keys to you.

FAQ

How do we get data to Patient Lookup?

​It depends on the type of data.  If Medicare data, we generally receive those data sets directly from Medicare.  If Medicare Advantage, commercial or Medicaid, we receive that data from you or the payor using SFTP (secure file transfer protocol).

RPM data is sent from the patient's home device to Patient Lookup using an API.

SDOH data is sent by the patient completing electronic questionnaires.

ADT data is sent from your selected ADT service provider to Patient Lookup.
